One in five hospital discharges is complicated by an adverse event within 30 days, which often leads to visits to the emergency departments and readmissions. IMPACT Act, a federal law, is now in effect and has resulted in reduced payments to hospitals with higher than average readmission rate for Acute myocardial infarction (AMI), heart failure and pneumonia patients. This session by expert speaker Sue Dill Calloway, RN, MSN, JD will discuss the impact of the federal law and how to lower hospital readmissions. It will also discuss what evidenced based studies exist and what are being planned in the future to reengineer the discharge process.
