On October 1, 2016 the CMS flexibility with ICD-10 coding and documentation will end. You will be required to provide specific codes that match your documentation, rather than the correct family of codes, for Medicare and other payers to accept the claim.
Practices submitting unspecified codes post October 1 can experience increased post payment audits and quality reporting errors. As audits increase, so will payer requests for medical records and clinical documentation. Failure to submit these will not only result in rejections, but an increased suspicion by auditors as well.
